FAQ: LPN Dental Coverage Change to EmblemHealth
1. What is changing regarding my dental plan?
Beginning January 1, 2025, EmblemHealth will be your new dental benefits provider and will administer your benefits through its Preferred Premier Plan.
2. Will my benefits remain the same?
Yes. There will be no changes to the dental benefits you currently receive.
3. Why is this change occurring?
The Fund and its Trustees are continuously working to enhance the quality of your benefits and the network of providers you can access. The EmblemHealth Preferred Premier dental plan offers improvements on both fronts.
4. Can I keep the same dentist I currently visit?
The EmblemHealth Preferred Premier network is closely equivalent to the United Healthcare Liberty network, so there’s a good chance your dentist participates in both. However, there are a small number of providers in the Liberty network who do not participate in the EmblemHealth Preferred Premier network. EmblemHealth is actively working to recruit the dentists that are not in its network.

5. What should I do if my current dentist does not participate with EmblemHealth Preferred Premier and I am in the middle of a treatment or procedure that will extend beyond December 31, 2024?
We have a plan in place for continuity of care, so you may continue to receive treatment from your current provider without interruption until the treatment is completed. Please contact the 1199SEIU Member Services Department at (646) 473-9200 for additional information.
6. Will I receive a new ID card?
Yes. EmblemHealth will send you a new ID card in mid-December 2024.
Note: Your ID card will be mailed in a plain, white, non-descript envelope from EmblemHealth (similar to how credit cards are mailed). Each eligible family member will receive their own ID card.
7. Whom do I call if I have questions about services rendered prior to January 1, 2025?

8. Whom do I call if I have questions about services rendered after January 1, 2025?
Please call EmblemHealth after December 31, 2024, at (800) 624-2414.
9. What should I do if I have a dental appointment in early January and I have not received my new ID card yet?
Please call EmblemHealth customer service at (800) 624-2414 to obtain your ID number. EmblemHealth customer service will also be able to assist you with obtaining your ID card and registering for their online member portal.
10. Where do I submit my dental claims from 2024?
For dental services performed prior to January 1, 2025, you (or your dentist) should submit claims to United Healthcare at PO Box 30567, Salt Lake City, UT 84130.
For dental Services performed on or after January 1, 2025, you (or your dentist) should submit claims to EmblemHealth at PO Box 2838, New York, NY 10016.
